In the fourth quarter of 2023, Columbiana Health and Rehabilitation, LLC stood out as the third largest nursing home in Shelby County, according to the Centers for Medicare and Medicaid Services (CMS).

Data shows that the nursing center had a total of 63 beds available, 68 less than the average in Shelby County as of the fourth quarter of 2023. They aided an average of 61.6 residents during that period.

Columbiana Health and Rehabilitation, LLC boasted an overall rating of 5 from the CMS, which is higher than the average of 3 recorded for nursing homes in Alabama.

During the quarter, The Healthcare Center at Buck Creek stood out as the largest nursing home in Shelby County, with 198 beds available. Shelby Ridge Nursing Home came in second place, with 131 beds available.

As of the fourth quarter of 2023, Columbiana Health and Rehabilitation, LLC achieved a zero-incident record for fines and penalties.

In total, there were three for-profit nursing homes in Shelby County, Alabama.
